Your success contributes to the thriving community we are building together. Qualifications: Licensed as a Registered Nurse in the applicable state; an associate degree is preferred. CPR certification is recommended, and experience in hospice, home health, or specialized nursing is advantageous. Willing to participate in an on-call rotation and possess reliable transportation with a valid driver's license and necessary liability insurance. Strong communication and organizational skills are crucial, along with proficiency in Microsoft Office Suite and EMR systems. This role requires regular physical activity, including lifting and transporting resources. Responsibilities: Conduct comprehensive nursing assessments and collaborate with the Interdisciplinary Group (IDG) to develop tailored care plans. Ensure accurate documentation and facilitate effective communication with the care team to enhance resource utilization. Provide education and emotional support to patients and caregivers, participating actively in on-call duties and care transitions. If serving as a Care Manager, coordinate all patient care aspects, including medication oversight and team collaboration.
