Eastern Time: 8:00 AM - 7:00 PM Other Time Zones: 7:00 AM - 6:00 PM, 8:00 AM - 7:00 PM, or 9:00 AM - 8:00 PM The Transitions of Care Nurse is instrumental in providing patient education, managing chronic conditions, and delivering personalized health coaching to members participating in various health improvement initiatives. Your Impact: Main Responsibilities: Perform thorough assessments to gauge each member's knowledge, skills, and needs. Develop and coordinate tailored health coaching plans based on assessment outcomes. Collaborate with healthcare providers to ensure successful execution of health coaching strategies. Educate members on clinical requirements and refer them to behavioral health professionals when needed. Employ motivational interviewing techniques to inspire health behavior changes. Evaluate the success of health interventions and make adjustments as necessary. Assist members in connecting to community resources, facilities, and appropriate healthcare providers. Guide members toward catastrophic case management as needed. Minimum Qualifications: Requires an Associate's degree in nursing and a minimum of 2 years of relevant clinical or home health/discharge planning experience, or a comparable combination of education and experience. A current and unrestricted RN license in applicable states is mandatory. Preferred Skills and Experience: A compact RN license is highly preferred. A Bachelor's degree in nursing is advantageous. Previous experience in case management is a plus. Bilingual proficiency in Spanish or Mandarin is highly valued. Experience in chronic disease management is preferred. A background in home health settings is desirable. For candidates working in-person or virtually in the specified locations, the salary range for this position is $65,600 to $113,160.
