Guided by the state Nurse Practice Act and agency standards, you will support clients with essential daily care, promote safety and comfort, and help them maintain dignity and independence in their home environment.What You Will Do Deliver personal care services such as bathing, grooming, skin care, dressing, toileting, transferring, and ambulation following the RN-developed care plan. Assist with household tasks including meal planning and preparation, feeding, light housekeeping, laundry, and bed-making. Measure and record vital signs including temperature, pulse, respiration, and blood pressure as ordered. Support therapy services by performing assigned range-of-motion exercises and other simple procedures. Assist clients in self-administration of medications as assigned and documented by the RN (CNA does not administer medications). Provide support with medical equipment such as wheelchairs, side rails, and footstools to ensure safety. Perform personal care services such as changing ostomy appliances or external catheter care per the care plan. Report changes in client condition or incidents immediately to the supervising RN. Maintain accurate, timely documentation and collaborate with schedulers to ensure an up-to-date care schedule. Participate in multidisciplinary case conferences and ongoing skill development and training. Minimum Qualifications High School diploma or GED. Completed a federally compliant CNA or aide training program and passed a competency evaluation. At least one year of experience as an aide in home care, hospice, or institutional care. Meets agency hiring criteria, including professional references. Ability to work independently while maintaining professional relationships with clients and families. Commitment to maintaining skills and adhering to agency policies, Code of Ethics, and Compliance Program. Valid driver's license and reliable transportation.
