Clinical Pharmacist (Texas-Licensed) Medicare CCM + 50-State Pharmacy/DME Licensing Lead $125,000+ DOE Company: The Wellness Company (TWC) / Wellness Care (Medicare Chronic Care Management Program) Location: On-Site , Richardson Texas. Occasional travel as needed. Role Summary We are seeking a Texas-licensed Pharmacist to deliver high-touch, consultative medication services for Medicare patients enrolled in Wellness Care's Chronic Care Management (CCM) program and to lead the licensing roadmap to build a national 50-state pharmacy and durable medical equipment (DME) operation. This is a clinical + operator role: you will directly impact patient outcomes while building the regulatory foundation for national scale. Key Responsibilities A) Medicare CCM Clinical Pharmacy Services (Patient-Facing) Medication reconciliation and optimization: Perform comprehensive medication reconciliation across all prescribers, pharmacies, and patient-reported products (Rx, OTC, supplements). Identify and resolve discrepancies (duplicates, omissions, incorrect dose/frequency, outdated therapies, drug-drug and drug-supplement interactions). Flag high-risk medications and age-related risks (falls, sedation, bleeding, anticholinergic burden) and recommend safer alternatives when appropriate. Coordinate with providers on regimen simplification, therapeutic interchange, and deprescribing opportunities. Side effect and safety counseling: Provide clear counseling on expected side effects vs. red-flag symptoms requiring urgent evaluation. Recommend practical mitigation strategies (dose timing, food/hydration guidance, OTC support where appropriate) and define escalation pathways. Educate patients and caregivers on safe storage, safe use of OTC medications, and common medication errors (missed dose actions, double-dosing prevention). Adherence, access, and affordability support: Assess adherence barriers (cognition, dexterity, access, cost, regimen complexity) and implement individualized adherence plans. Recommend refill synchronization, reminder systems, and packaging solutions (when available). Support navigation of formulary alternatives and prior authorization steps in partnership with clinical and administrative staff. Clinical monitoring and care coordination: Recommend monitoring parameters (vitals, symptom checks, lab follow-up where clinically relevant) and coordinate follow-ups with the care team. Document interventions and outcomes in the patient record to support continuity of care and CCM requirements. Participate in case conferences and continuous improvement of patient education scripts, protocols, and escalation criteria. B) 50-State Pharmacy + DME Licensing Lead (Build & Compliance) Own and execute the 50-state pharmacy licensure strategy (nonresident pharmacy registrations, renewals, and change filings). Develop and maintain a master licensing tracker (state requirements, fees, timelines, renewal calendars, responsible parties, and documentation repository). Partner with Legal/Compliance to draft and maintain required policies and SOPs (patient counseling standards, complaint handling, recalls, record retention, QA/QC). Coordinate facility readiness for inspections (controlled substances policies where applicable, security controls, temperature logs, inventory management, recordkeeping). Lead the regulatory build for DME operations, including state supplier requirements and documentation needed for audits and accreditation pathways as applicable. Collaborate with Operations and Technology to ensure workflows support compliance (documentation fields, counseling logs, audit trails, role-based access controls). Create training materials for care coordinators and support staff on medication workflows, documentation standards, and safety escalations. Success Metrics Reduction in medication list discrepancies and resolution of high-risk therapy issues within defined service levels. High patient satisfaction with pharmacist consults (CSAT/NPS) and measurable adherence improvements (refill consistency, reduced gaps). On-time completion of priority state licenses and renewals; inspection readiness supported by a complete SOP library and evidence logs. Operationally usable licensing tracker and compliance cadence adopted across teams (clear owners, timelines, and audit readiness). Required Qualifications Active, unrestricted Pharmacist license in Texas (required). Willingness to obtain pharmacist licensure in all 50 states (company covers fees and administrative costs; you must complete CE and application requirements). PharmD or BS Pharm from an accredited program. Experience supporting older adults and chronic disease patients; strong competency in polypharmacy risk management. Excellent patient and caregiver communication skills, including senior-friendly counseling. Strong documentation habits and comfort working in structured clinical workflows (EHR/CRM). Highly organized operator with the ability to manage multi-state licensing projects and deadlines. Preferred / Bonus Qualifications Active pharmacist licenses in additional states. Experience with Medicare populations, MTM, transitions of care, CCM/RPM support, or long-term care pharmacy. Prior involvement in pharmacy licensing, audits, inspections, or compliance program buildout. Familiarity with DME workflows, supplier compliance, and/or accreditation processes (helpful but not required).
