The Home Health Aide plays a crucial role in delivering personalized care to patients in a compassionate manner. You will provide essential services that enhance comfort and dignity, as well as document observations and care according to our high standards and regulations. Key Responsibilities Care Management and Service Delivery - 95% Deliver psychosocial support and personal care, including bathing, oral hygiene, grooming, and dressing. Assist patients with mobility by supporting walking, transferring, and positioning, utilizing devices when necessary. Guide patients through prescribed exercises to ensure adherence to their care plans. Support toileting needs, including the use of bedpans and incontinence care. Assist in meal preparation and dietary needs, including cleaning the kitchen area afterwards. Help patients with medication administration, providing reminders and assistance as needed. Perform light housekeeping and accompany patients to medical appointments as needed. Execute additional tasks as assigned, such as monitoring vital signs and adhering to infection control procedures. Provide transportation for patients to appointments or activities as required. Administration and Reporting - 5% Observe and document patient status using our electronic medical record (EMR) system, ensuring accurate care reporting. Additional Duties Other responsibilities may be assigned as needed. Qualifications Education High school diploma or equivalent is required. Must be a Certified Nursing Assistant (CNA) in good standing with the Ohio Department of Health Nurse Aide Registry, or have successfully completed the Medicare competency evaluation program for home health aides. Experience Preferred: Minimum of one year experience as a home health aide in a Medicare-certified agency. Preferred: Familiarity with home health aide best practices and procedures. Other Requirements Fluency in English, both written and spoken. Possession of a valid driver's license and an acceptable driving history; must meet insurance underwriting requirements. Working Conditions Sitting: Up to 8 hours/day Standing: Up to 4 hours/day Walking: Up to 2 hours/day Lifting and moving patients or equipment may reach up to 50 pounds. Driving time can be up to 6 hours/day. Availability during weekends, evenings, and holidays as scheduled. Exposed to various working conditions, including potential risks associated with patient care. May be exposed to infectious materials during the course of duties. This position entails exposure to bloodborne diseases (Risk Category I).
