Click Apply Now! - Your Responsibilities!Conduct Nursing and/or Needs Assessment at the start of careConduct meet and greets for new field staff placementsMonitor and coach home nursing and home services field staffReview nursing notes and coach field staff to improve qualityReview weekly care notes for quality of documentation and changes in patient condition requiring revisions to the Plan of Care.Nurse Case management: contact families, coordinate care, field visits – home and facility, communicate changes in Plan of Care.
