Job Description

Join a dedicated healthcare team providing compassionate hospice care in a supportive environment within the Midwest region. This part-time Registered Nurse (RN) role involves delivering direct patient care using the nursing process, demonstrating clinical expertise, and adhering to professional ethical standards. Essential Duties and Responsibilities Participate in agency councils, committees, or serve as a clinical coach, unless currently enrolled in a nursing degree program. Administer medications and perform planned interventions safely and efficiently, following established policies and protocols. Communicate patient condition changes to physicians using the SBAR format and advocate for patient needs collaboratively. Coordinate patient care through effective teamwork with healthcare professionals and facilities. Adapt positively to the dynamic healthcare environment and document patient care activities accurately using electronic medical records. Evaluate patient progress toward goals and update care plans accordingly. Develop age-appropriate, patient-centered care plans with patient and family involvement upon admission. Provide education tailored to patient and family needs, considering cultural backgrounds and education levels. Engage in self-reflection to improve performance and apply critical thinking in patient assessment, care planning, and discharge preparation. Maintain up-to-date knowledge and skills through evidence-based practice relevant to hospice nursing. Education Graduation from an accredited nursing school is required. A Bachelor of Science in Nursing is preferred. Experience At least one year of clinical nursing experience is required, with preference given to candidates with home health nursing experience. Certification and License Current RN license valid in the state or through compact licensure is required unless a new graduate, who must obtain licensure within 90 days. Basic Life Support certification from the American Heart Association is mandatory. Mental and Physical Requirements This role demands considerable mental focus due to the sensitive nature of end-of-life care. Physical activities include lifting up to 35 lbs., frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching. Working Conditions Frequent exposure to communicable and infectious diseases, potential contact with hostile individuals, and handling of biohazardous materials are part of the work environment. Nurses will encounter varying environmental temperatures and diverse community settings.
