Position Overview: Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards. Why Join Us? ~ Competitive Compensation: $65|hour ~ Comprehensive Benefits: Sick leave ~ Work Schedule: Monday - Friday (8:00 AM - 4:30 PM, Day Shift) ~ Professional Growth: Academic medical center exposure Qualifications: ~ Education: ADN|BSN from an accredited program ~ Licensure: Current and active New York State Registered Nurse (RN) license is required ~ Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain prior to employment ~ Experience: ~ Minimum of 1 year of recent experience (within the last 2 years) in an acute care hospital of similar size and complexity ~ Proven experience in hospital case management within an acute care environment ~ Preferred: Previous Emergency Department Case Management (ED CM) experience strongly desirable ~ Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system Key Responsibilities: ~ Conduct utilization reviews for appropriate care level and coverage ~ Coordinate discharge planning and post-acute care transitions ~ Document care coordination in EPIC ~ Ensure compliance with payer guidelines and regulations ~ Collaborate with ED teams for rapid case management ~ Analyze resource utilization trends and recommend improvements
