DetailsClient Name Providence Regional Medical Center Everett Colby Campus Job Type Travel Offering Nursing Profession Registered Nurse (RN) Specialty Case Management Job ID 36012859 Job Title Registered Nurse (RN) - Case Management Weekly Pay $2257.78 Shift DetailsShift Days - 5 x 8 Scheduled Hours 40 Job Order DetailsStart Date 03/30/2026 End Date 06/29/2026 Duration 13 Week(s) Job Description Title Acute Care Case Manager - Care Coordination / Discharge Planning Schedule Shift: 5x8 Days Guaranteed hours: 72 hours per 2-week period Weekend requirement: Every other weekend Assignment Details Locations: Both Colby & Pacific campuses Patient ratio experience requested: 25-30 Requirements - Minimum 1 year of acute care experience - RN licensure - BLS certification - Experience in discharge planning and care coordination - Diverse case management experience preferred (Med Surg, LLOS, ED) - Experience with patient populations: Neonates, Infants, Pediatrics, Adolescents, Adults, Geriatrics - EPIC charting experience preferred Roles & Responsibilities - Coordinate patient care through discharge planning within an acute care setting. - Collaborate with interdisciplinary teams, including social services, to optimize patient outcomes. - Assess patient needs and develop individualized care plans. - Manage the transition of care for patients across different hospital departments and locations. - Facilitate DME ordering as needed and ensure patient follow-up post-discharge. - Float between hospital areas as required to provide care coordination and support. Compliance / Notes - Include all RTO requests at time of submittal. - Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day). - Facility-paid parking. - Scrub color: Any (no reddish-orange permitted).
