Department Description: Want to have every other week off? We are seeking a full time On Call Visit RN to work closely with the triage nurse team to support clients in the home setting from 5pm to 8am. The Community Hospice team delivers care to patients in the community, providing a wide range of skilled nursing care and support. We are an interdisciplinary team all collaborating together to support patients to remain in the setting of their choice, their home. The first 5 hours you will have intentional work such as admissions, the rest of the time you just need to be available for any urgent calls! RN II and RN IIIs are considered for this position. Minimum Requirements: - Associate's degree in nursing or higher. - Minimum of four years of experience as Community Hospice Nurse or minimum of five years of experience in home health nursing. - Candidate must live within reasonable driving distance to our service area as you will be making on-site visits at patient's homes in Chittenden and Grand Isle counties. - Certification in hospice and palliative care preferred.
