Job Description

A skilled Occupational Therapist opportunity is available to provide in-home patient care in Irrigon, OR. This contract position involves working closely with the interdisciplinary team to assess and implement therapeutic interventions designed to help patients regain independence in daily activities such as self-care, leisure, and work. The role reports to the Home Health and Hospice Manager and requires use of your own vehicle for travel to patient homes. Key Responsibilities: Evaluate patients' functional status and interpret physician's plan of care. Conduct assessments and reassessments using objective tests and measurements. Collaborate and communicate with team members to coordinate care plans. Educate and guide patients and colleagues as needed. Provide compassionate care, respecting patient rights and privacy. Document all treatment activities and communications accurately. Respond appropriately to changes in patient conditions and emergency situations. Support organizational mission, policies, and regulatory requirements. May assist in other departments qualified for patient care if needed. Qualifications: Graduate of an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation and the American Occupational Therapy Association. Licensed as an Occupational Therapist in the state of Oregon. Eligible for or registered with the American Occupational Therapy Association is preferred. Valid driver's license and proof of auto insurance required. Current Basic Life Support (BLS) certification. Continuing education relevant to occupational therapy, particularly in physical disabilities, rehabilitation, mental health, and home health care is desired.
