Provide therapeutic services that help patients regain function in daily activities as an Occupational Therapist delivering home health care in Hermiston, OR. This contract position involves patient care in home settings, requiring reliable personal transportation. Responsibilities: Evaluate patients' functional status and implement physicians' plans of care. Conduct thorough assessments and reassessments using objective tests and measurements. Collaborate as part of an interdisciplinary team through communication and team meetings. Educate and guide patients and coworkers appropriately. Maintain accurate, timely documentation of all treatment and coordination activities. Respond to changes in patient conditions and adapt interventions accordingly.
