Job Title: Home Health Physical TherapistLocation:Martinez, CA Job Description:Join our team as a Home Health Physical Therapist, focusing on geriatric care. This 13• week contract, with potential extension, runs Monday to Friday, 8:00 am to 5:30 pm. Responsibilities:• Provide quality physical therapy to home health patients. • Manage a caseload of about 6 patients daily. • Schedule and coordinate patient visits. • Collaborate with RNs and OT/PT staff. • Use EMR systems like Oasis and HCHB (training available). Requirements:• Valid CA PT license. • BLS certification. • At least 1 year of home health experience. • Preferably experienced with geriatric patients.
