Job Title: Licensed Practical Nurse (LPN/LVN) - Home Health Work Location: Rolla, Missouri, USA 65401 Hours per Day: 8 Hours per Week: 40 Shift: Monday to Friday until shift completion On-Call Requirements: Yes, 1 day per week, 1 weekend per month Travel Area: Osage, Maries, Gasconade, Franklin, Crawford, Pulaski, Phelps, Dent, Reynolds Guaranteed Hours: 32 hours per week Productivity Units: 30 units Job Description: The primary function of the LPN/LVN is to administer skilled nursing care to patients in their place of residence, as prescribed by the physician and in accordance with the established plan of care. The LPN/LVN works under the supervision of a Registered Nurse (RN) and coordinates care with the interdisciplinary team as well as the patient/family. The LPN/LVN will deliver care in compliance with State, Federal, and Medicare standards. Skills and Qualifications: Current Nursing License in the Practicing State Valid Driver's License Current CPR Certification Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice settings Proof of current Hepatitis consent/declination and TB vaccination record Ability to use electronic medical charting and time-keeping software (Homecare Homebase) Reliable transportation and current auto liability insurance coverage Must read, write, and speak fluent English Must have good and regular attendance Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing Preferred Qualifications: Homecare Homebase experience preferred If you are interested please share an Updated Resume to get your profile shortlisted.
