RN Case Manager position in Glenwood Springs, CO

Job Details - Position - RN Case Manager Location - Glenwood Springs, CO 81601 Shift - 3-12's or 4-10's YOU CHOOSE! This is a full-time day position with weekend rotation, no call. Guaranteed Hours - 36 hrs Duration - 13 weeks Gross Weekly - $2,900/wk for 40hrs Note - RADIUS RULE: Traveler's perm address must be at least 75 miles away from our facility. Graduate of an accredited or state board of nursing approved Minimum of 3 year's RN Case Management Experience is required. Duties - Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum. Conducting intake assessments with patients and families. Ensuring that families feel sufficiently equipped and supported. Developing individualized treatment plans that account for physical, financial, and other constraints. Adjusting treatment protocols, as needed. Suggesting and coordinating appropriate follow-up services to promote wellness. Ensuring that patients have transport to and from their homes and all pertinent healthcare facilities. Meeting with members of healthcare teams to discuss patients' progress, and to deliberate on appropriate care. Checking to see whether patients have sufficient insurance coverage or finances to pay for their treatment. Devising innovative solutions or suggesting appropriate referrals for patients who cannot afford to access treatment.
