In this role, the LPN will provide direct nursing care to patients in their homes under the supervision of a Registered Nurse or physician. The ideal candidate has prior home healthcare experience, strong clinical skills, and a passion for delivering high-quality, patient-centered care in a home setting.This position requires travel to patient homes throughout Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties.ResponsibilitiesProvide skilled nursing care to patients in their homes in accordance with the plan of careAssist with patient assessments and report findings to the supervising RNAdminister medications and treatments as prescribedMonitor and document patient conditions, progress, and responses to careObserve and report any changes in patient condition to the RN or supervisorEducate patients and families on medications, treatments, and daily care needsMaintain accurate and timely documentation in compliance with agency and state requirementsFollow all infection control, safety, and home healthcare protocolsQualificationsCurrent Licensed Practical Nurse (LPN) license in the state of GeorgiaPrevious home healthcare experience REQUIREDKnowledge of basic nursing practices and home health proceduresExcellent interpersonal, communication, and patient care skillsAbility and willingness to travel to Atlanta, Cobb, Gwinnett, Henry, and Clayton CountiesValid driver’s license and reliable transportationCurrent First Aid and CPR certificationStrong organizational skills and attention to detailFlexible work from home options available.
