As the Licensed Practical Nurse, you will play a pivotal role in individuals' lives, making a significant impact through: Compile monthly/quarterly statistics and submit them to the Health Care Coordinator. Be available for after hour consultations regarding medical concerns. Enter all medical information from all doctor appointments onto the computer as well as into the CONNEX system. Document all medical information in the client's medical chart. Send all medical information in to youth's respective counties in accordance with our current contracts (ie: email to Albany County.) When clients return from the appointment, note and schedule any lab tests, x-rays, referrals and follow-up on appointments. Make Residential Manager aware of all appointments to ensure proper staffing and transportation. Must keep an inventory of all medical supplies and keep the medical closet organized. Provide proper supervision to ensure the health, welfare and safety of clients. Provide transportation on an as needed basis.
