Position Responsibilities: The Care Coordinator 2 utilizes various strategies to manage aspects of a member's physical, environmental, and psycho-social health. Develop a trusting, long-term relationship with each member to enhance care quality and coordination. Coordinate all required medical and non-medical services, addressing functional, social, and environmental needs. Collaborate with the Service Coordinator and Transition Coordinator to meet members' identified needs. Work with all Medicare payers and providers to ensure seamless care for members eligible for both Medicaid and Medicare services. Act as the primary contact for the Interdisciplinary Care Team, ensuring that all member needs are met through effective communication with team members and external resources. Your Skills Matter Required Qualifications: Active Registered Nurse (RN) license in Indiana with no restrictions A minimum of two years of clinical experience in case management or similar healthcare services Intermediate to advanced computer skills including proficiency with Microsoft Word, Excel, and Outlook Exceptional communication skills, fostering rapport with diverse stakeholders Strong critical thinking, organization, and problem-solving abilities Capacity to prioritize in a fast-paced work environment Experience with electronic medical records and various applications/software Must reside in Indiana Preferred Qualifications: Bilingual in English/Spanish or English/Burmese Prior experience in nursing home diversion, long-term care, or case management Experience managing Home and Community Based Services waivers Previous work with Medicare & Medicaid recipients Experience with senior populations and health promotion initiatives Familiarity with community health and social service agencies Additional Information: About Humana: Your growth fuels our mission at Humana. Hours: Standard 40-hour work week, Monday through Friday 8:00 AM to 5:00 PM; overtime may be required as needed. Travel: Must be willing to travel approximately 70% to meet members. On Call: Telephonic on-call for occasional night/weekend responsibilities may be required. Technical Requirements: Self-provided internet service should include a minimum of 25 Mbps download and 10 Mbps upload speed; wired connections preferred. Satellite, cellular, and microwave connections require prior approval. Humana will supply necessary phone equipment to meet job requirements. Work from an area free from distractions to ensure protection of member PHI/HIPAA information. Driver's License and Insurance: As a part of Humana's Driver Safety program, you must possess: A valid state driver’s license Proof of personal vehicle liability insurance with minimum limits of $100,000/$300,000/$100,000 Access to a reliable vehicle Tuberculosis (TB) Screening: This role requires participation in Humana's TB screening program for patient-facing roles. Interview Process: Our hiring process includes a screening technology that enhances decision-making by allowing you to share information about your experience at your convenience through text, video, or voice formats. This position may require occasional travel to Humana offices for training or meetings. Scheduled Weekly Hours: 40 Compensation: The estimated salary range for this full-time position is $71,100 - $97,800 per year. Compensation may vary based on geographic location and individual qualifications. This role is also eligible for a performance-based bonus incentive plan.
