We are looking for a passionate Clinical Manager to lead and coordinate direct patient care services provided by our exceptional clinical team. This pivotal role ensures the delivery of high-quality home health care. Your responsibilities will include: Strategically planning and managing clinical operations to enhance patient care quality. Conducting thorough patient assessments and ensuring care plans are tailored to meet individual patient needs. Supervising caregiver assignments and overseeing the overall delivery of care to patients. Reviewing patient cases to assess health or hospice needs and verifying admission criteria compliance. Guiding clinicians in achieving therapeutic goals and aiding in the development of personalized care plans. Monitoring adherence to documentation standards and regulatory requirements, ensuring timely billing and audits. Fostering effective communication among care teams, physicians, and caregivers to optimize patient outcomes. Collaborating with management to establish revenue and budgeting goals for the organization. Engaging in marketing initiatives to promote our services and reach more patients. Overseeing clinical staff, providing leadership, performance evaluations, and addressing any necessary corrective actions. Participating in the recruitment, hiring, and training of clinical staff to maintain high performance standards. Assessing training needs for staff and conducting educational programs to enhance their skills. Identifying areas for improvement and implementing strategies to elevate performance and patient care. Reviewing clinical staff documentation to ensure accuracy and compliance with care protocols. Leading quality assurance and performance improvement initiatives to enhance patient outcomes. Providing direct patient care during emergencies when necessary. Acting as Branch Director in the absence of management, ensuring compliance with all standards. Pursuing professional development and staying informed about healthcare regulations. Building relationships with community referral sources and participating in care-related programs. Executing other related duties as assigned. To excel in this role, you should possess: A degree from an accredited School of Nursing. A current state license as a Registered Nurse. CPR certification. A valid driver's license, auto insurance, and reliable transportation. Minimum of two years of nursing experience, including at least one year in a management role within home care or hospice settings. Scheduled Weekly Hours: 40 Compensation: The annual salary for this role ranges from $114,300 to $157,200, depending on experience and location. This position also qualifies for a performance-based bonus incentive plan.
