Job Details: Job Title: LPN/LVN - LTC Job Location: 98 Hospitality Dr, Barre, VT 05641 Facility Name: Barre Berlin Health & Rehab Center Contract Duration: 13 week contract Shift: Variable Evenings/Night Shift | 5×8 Hours Hours: 40/week Position Summary: We are seeking a dedicated LPN/LVN to join a reputable Long-Term Care (LTC) facility. The ideal candidate should have strong clinical skills, excellent bedside manner, and the ability to work as part of a multidisciplinary care team to provide safe, compassionate, and high-quality care to residents. Key Responsibilities Provide direct nursing care to residents in accordance with facility policies and state regulations. Administer medications, treatments, and injections as prescribed. Monitor patient conditions, record vital signs, and report any changes to the RN or physician. Assist with activities of daily living (ADLs) and support resident comfort and mobility. Maintain accurate and timely documentation in medical records. Collaborate with RNs, CNAs, and other healthcare staff to ensure coordinated care. Provide emotional support and build trust with residents and their families. Adhere to infection control, safety protocols, and facility guidelines. Requirements Active LPN/LVN license (State of Massachusetts). Minimum 1 year of LPN/LVN experience, preferably in Long-Term Care. Current BLS certification (AHA preferred).
