SUMMARY: Mend Health is on the lookout for a compassionate and skilled Home Care Nurse Practitioner. As a Nurse Practitioner focusing on Care Management, including transitional and chronic care, you will play an essential role in providing exceptional healthcare services to our patients in a variety of home environments. Your responsibilities will include assessing, treating, and managing acute and chronic illnesses while working closely with patients, families, and our multidisciplinary team to promote recovery and enhance patient well-being. KEY RESPONSIBILITIES: Perform focused or comprehensive assessments for post-acute patients, recognizing chronic conditions like heart disease, diabetes, and COPD. Identify collaboration opportunities to enhance clinical integration with patients and other health systems, ultimately improving clinical and operational outcomes. Provide guidance, support, and education to clinical nurses and medical assistants. Educate patients, families, and caregivers on care management practices, motivational interviewing techniques, preventive strategies, and recognizing signs of complications. Collaborate effectively with physicians, specialists, and other healthcare providers to ensure coordinated patient care and optimized treatment outcomes. Monitor care management performance to ensure compliance with medical and clinical goals. Maintain accurate, up-to-date medical records, including wound assessments, treatment plans, and progress documentation. Engage in continuous education and training to remain current in Chronic Care Management and technologies. Work in partnership with the Medical Director and key staff to develop and implement enhancements to care models. Adhere to Mend Health's policies, procedures, and regulatory standards, ensuring compliance with best practices and patient safety protocols. Contribute to the leadership team and actively participate in the Company's growth strategy. QUALIFICATIONS: 3-5 years of clinical experience as a Certified Nurse Practitioner (CNP) in direct patient care, particularly with medically complex patients utilizing multimodal therapies. Wound care experience is preferred. Demonstrated proficiency in assessments and a deep understanding of chronic illness processes. Excellent interpersonal and communication skills, with the ability to educate and empower patients and caregivers. Capacity to work both independently and collaboratively within a multidisciplinary team. A compassionate, patient-centered approach with a strong commitment to delivering high-quality healthcare services. EDUCATION AND CREDENTIALS: Master's degree in nursing from an accredited Nurse Practitioner program. Active, unrestricted Certified Nurse Practitioner (CNP) license in the state of practice, along with national certification as a Nurse Practitioner in Adult-Gerontology, Family, or Acute Care.
