Join a Dynamic Team Where Your Expertise Makes a Difference! This is not a clinic-based position. As a Home Dialysis Nurse (HHD/PD), you will provide personalized, one-on-one care and education directly in patients' homes, empowering them to manage their dialysis therapies with confidence and dignity. As a trusted clinical expert, you'll work to ensure quality outcomes, regulatory compliance, and continuity of care in home hemodialysis and peritoneal dialysis programs. We are expanding our team in Florida and you will focus on serving one of five designated territories: Palm Beach County, Clearwater/St. Petersburg, Broward County, Tampa, or Space Coast. About This Role This position combines advanced clinical nursing practices with patient education and independent fieldwork. You will assess patient readiness, deliver training, monitor outcomes, and collaborate closely with nephrologists and interdisciplinary teams. Enjoy autonomy in the field while staying connected to a supportive clinical leadership team. This role requires sound judgment, regulatory knowledge, and comfort operating in patient home environments. Your Key Responsibilities Clinical Assessment & Care Management: Conduct thorough assessments for patients entering HHD and/or PD programs; evaluate patient readiness, medical stability, and adequacy of home environments; collaborate with nephrologists to develop and manage customized Plans of Care. Patient Home Training & Ongoing Support: Deliver in-home training on HHD and PD therapies, ensuring patients and caregivers are competent in equipment usage, infection control, troubleshooting, and emergency procedures. Quality, Safety & Regulatory Compliance: Maintain compliance with CMS ESRD guidelines and company policies; participate in Quality Assessment and Performance Improvement (QAPI) activities; ensure accurate and timely clinical documentation. Staff Training & Clinical Leadership: Provide training and support to nursing staff involved in home dialysis services; act as a subject matter expert for protocols and preventions related to infection and patient education. Emergency Preparedness & Continuity of Care: Educate patients and caregivers on emergency procedures and maintain communication during critical situations. Nature of Work: This professional-level role requires independent clinical judgment and decision-making while interpreting complex medical information and providing patient education. Required Qualifications: Active, unrestricted Registered Nurse (RN) license in Florida Minimum of 1 year of home dialysis experience (HHD and/or PD) Strong understanding of CMS ESRD Conditions for Coverage Exceptional clinical assessment and patient education skills Valid Florida Driver's License and willingness to travel to patient homes Current BLS certification Preferred Qualifications: Experience with QAPI or survey readiness activities Prior experience training or mentoring nursing staff Comprehensive Benefits Package: Health & Wellness: Medical, Dental, and Vision Insurance; Paid Time Off (PTO); Paid Holidays.
