Our interdisciplinary team works collaboratively to reduce symptom burden and enhance quality of life for patients and their loved ones.If serving individuals on their end-of-life journey resonates with you, we would love to welcome you to Vitality.Schedule: Per Diem — Every Other Saturday & SundayResponsibilitiesAs a Hospice Visit Nurse, you will: Provide professional nursing care, including admissions, initial comprehensive assessments, and ongoing evaluations.Develop, implement, and update individualized plans of care based on patient needs.Collaborate closely with the hospice physician and full Interdisciplinary Team to ensure appropriate interventions.Oversee and support the work of home health aides, social workers, and chaplains.Educate patients and caregivers to promote safety, comfort, and understanding.Maintain high standards of documentation, compliance, and clinical excellence.Contribute to continuous improvement efforts to elevate the quality of care across the agency.
