Provides assistance to co-workers and promotes teamwork by sharing information useful to achieving desired results.Ensures that all residents are treated fairly, with kindness, dignity, and respect, and their rights are protected at all timesRespects and upholds all residents' personal and property rights whenever carrying out duties, including knocking before entering a resident's room.Complies with all company and departmental policies and procedures.Participates in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.Ensures that care plan includes measurable objectives and timetables to meet resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in assessment, and that schedule of activities is developed as required. KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS 1-3 years' experience providing direct care to multiple patients/residents, one years' previous MDS experience in Skilled Nursing environment preferredNursing degree from accredited college or graduation from approved LVN program required, RN preferredMust possess current, unencumbered license to practice as an LVN or RN in California.Current valid CPR certificationKnowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participationCommitment to the company mission to consistently deliver high quality, person-centered care with dignity, respect, compassion and integrity, and to enrich every life we touch.
