Position Overview We are seeking a talented Occupational Therapist to join our team. In this PRN role, you will deliver skilled occupational therapy services to home health and outpatient clients, ensuring the highest quality care in line with physician orders. Key Responsibilities: Assist the physician in evaluating client function levels through diagnostic methods. Conduct initial and ongoing comprehensive assessments, including OASIS assessments. Evaluate the client’s environment and support system for effective care planning. Implement and adjust interdisciplinary care plans in collaboration with physicians. Guide clients in therapeutic self-care and activities to enhance their independence and functioning. Educate clients on the use of mobility aids and adaptive devices. Provide detailed instruction for the client and their family about the Occupational Therapy process. Set treatment goals based on client assessments and conduct re-evaluations as necessary. Coordinate outpatient services for procedures that cannot be performed at home. Assess the effectiveness of therapy and make necessary adjustments to the care plan. Document clinical notes on the day of the visit, ensuring chart updates within 72 hours. Maintain regular communication with physicians regarding client progress and needs. Collaborate with team members to educate them on Occupational Therapy techniques for client care. Oversee Home Health Aides and Occupational Therapy Assistants as required. Engage in agency meetings and professional development opportunities. Confirm weekly visit schedules with the Director of Nursing to ensure coordinated care. Participate in discharge planning and complete Occupational Therapy Discharge Summaries. Commit to continuous professional development through workshops and education. Adhere to agency policies and engage in quality improvement activities. Attend scheduled patient care conferences. Complete OASIS assessments and related documentation timely. Gather comprehensive medical histories related to the client’s conditions. Evaluate support from family and community services required for client care. Ensure that physician orders are appropriate and suggest modifications as needed. Qualifications: Graduate of an accredited Occupational Therapy program recognized by the American Occupational Therapy Association. Preferred membership in the American Occupational Therapy Association. Current state licensure as an Occupational Therapist. Minimum of one year of institutional experience.
