This role involves assisting individuals with daily living activities, ensuring their comfort, safety, and well-being in a supportive home environment.Responsibilities Provide personal care services such as bathing, dressing, grooming, and toileting assistance.Monitor and report changes in client health and behavior to medical staff and family members.Assist with medication reminders and ensure proper adherence to prescribed treatments.Support mobility and physical therapy exercises as directed by healthcare professionals.Perform light housekeeping tasks to maintain a clean and safe living environment for clients.Offer companionship and emotional support to enhance clients' quality of life. Requirements Valid CNA certification or Home Health Aide training.Demonstrated experience providing personal care in a home health setting preferred.Strong communication and interpersonal skills with the ability to connect with clients of all ages.Reliable transportation and the ability to travel to client homes in Fort Wayne, IN.Compassionate, patient, and committed to maintaining client dignity and privacy.Ability to follow care plans and adhere to safety protocols.
