Job Summary As a Registered Nurse at VitalCaring, you will play a crucial role in planning and delivering compassionate care focused on providing comfort, dignity, and quality of life. You will support patients and families through excellent clinical care, expert symptom management, and essential education, guided by our core values of trust, capability, compassion, proactivity, and a calling to serve. Each interaction is a chance to create a meaningful difference. Essential Functions Patient Care Conduct initial and ongoing comprehensive assessments of patients and families to identify hospice needs, including detailed physical assessments and history review. Provide nursing care using comprehensive nursing processes in compliance with the professional practice act. Assess and evaluate patient status, initiate and adjust personalized plans of care, and adapt to any changes in conditions. Utilize health assessment data to determine nursing diagnoses and establish patient-centered palliative goals. Implement preventive and rehabilitative nursing procedures; administer prescribed medications and treatments. Counsel and educate patients and caregivers on care needs, symptom management, medication, diet, and safety. Assist patients with daily living activities and promote their independence and comfort whenever feasible. Act as Case Manager when assigned, coordinating all facets of patient and family care. Communication & Coordination Document timely and accurately clinical notes that reflect patient conditions, care provided, and outcomes related to pain/symptom management. Communicate effectively with physicians regarding patient needs and changes in conditions; obtain and fulfill required medical orders. Collaborate with community partners and interdisciplinary team members to ensure continuity and quality of care. Support the emotional needs of patients and families; maintain a safe environment aligned with hospice principles. Participate in interdisciplinary group (IDG) meetings to aid in coordinated care planning. Additional Responsibilities Participate in on-call rotations as per agency policy. Ensure arrangements for necessary medical equipment and services are in place. Supervise and delegate responsibilities effectively to ancillary personnel. Engage in continuous professional development through education and training sessions. Complete all assigned case responsibilities. Involve in quality assessment and performance improvement initiatives. Qualifications Graduation from an accredited school of nursing with 1-2 years of recent acute care experience Current RN licensure in the state of practice and current CPR certification. Bachelor's degree preferred, with one year of hospice experience ideal. Must possess a valid driver's license with a reliable and insured vehicle.
