Title: Temp - PT - Home Health (Days) Pittsfield, MA HEALTH SYSTEM WILL NOT CONSIDER ANY TRAVELER WHO RESIDES WITHIN 50 MILES OF FACILITY Description THIS IS A HOME HEALTH NEED! The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient's comprehensive plan of care. Rehabilitation equipment including but not limited to: splints, orthoses prostheses, wheelchairs, ADL equipment Modalities. Candidate required to be Home Health Certified Requirements 2+ acute nursing care (hospital base) and Home Care experience Valid Driver License required for this need Must be able to work independently with minimal supervision/assistance Acute care experience preferred Certified home health experience including proficiency with OASIS required and NON-NEGOTIABLE Certification Requirements BLS State License Requirements Massachusetts
