Key Responsibilities Provide comprehensive outpatient care to adult and senior patients. Accurately maintain medical records and transition documentation promptly. Diagnose and manage a range of complex medical conditions. Coordinate patient referrals and collaborate with specialists to ensure continuity of care. Engage in clinical quality improvement initiatives. Work within interdisciplinary teams to deliver holistic patient care. Support strategic initiatives aimed at enhancing care delivery. Exercising independent clinical judgment in patient management. Required Qualifications MD or DO from an accredited medical school. Active and unrestricted medical license. Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine. A minimum of 2 years of experience in value-based care or with high-acuity geriatric patients. Commitment to enhancing patient experience and health outcomes. Participation in a Tuberculosis (TB) screening program. Preferred Qualifications Specialty training in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics. Experience with senior populations or in value-based care settings. Proficiency with electronic health records and digital documentation. Strong interpersonal, collaboration, and communication skills. Able to work independently and adapt to changing clinical environments. Experience supervising Advanced Practice Providers (NPs/PAs). Bilingual proficiency in English and Spanish.
