DescriptionFLSA Employment Status: Nonexempt/Hourly JOB SUMMARY To provide quality case management and psycho therapeutic services to all nursing home residents. Serves as a member of the interdisciplinary team supporting the facility's treatment program and philosophy. Reporting Structure: Reports to the Nursing Home Administrator MINIMUM QUALIFICATION REQUIREMENTSEducation High School Diploma or GED Equivalent Graduate of an accredited LPN program Work Experience Must have two (2) years' experience in social work or case management. Nursing Home experience is preferred. License/Certification Current Tennessee State Licensed Practical Nurse with no restrictions required Current Tennessee Driver's License required Current Basic Life Support (BLS) CORE COMPETENCIES Mission, Vision Core Values/Standards of Conduct AIDET/Organizational Expectations Safety Quality Flexibility Communications Customer Service Diversity and Inclusion Finance JOB SPECIFIC CORE COMPETENCIES Nursing Home Elderly Services Social Services Resources Restraints (Safety) Electronic Health Records Abuse and Neglect of Adult Patient Acute Coronary Syndrome/Chest Pain Protocols Biohazard Waste CC Hand Hygiene Department Specific Specialty Area ESSENTIAL FUNCTIONS Developing treatment plans in collaboration with the HMC nursing home team communicating pertinent information to family, payer, and/or external case managers. Provides psychosocial assessment and substance abuse/chemical dependency evaluation of patients, and crisis intervention as necessary. Provides social services and resources for the nursing home residents. Develops and coordinates an individualized discharge plan for the residents and maintains an awareness of funding sources and needs. Completes MDS required assessments and charts completion of the assessments. Monitors the residents for any social related issues. Keeps staff informed via morning meeting of any major social issues, discharges, and level changes. Contacts family/ responsible party when therapy puts out the resident's discharge notice, informing them of date of discharge. Completes appropriate ABNs paperwork. Schedules and coordinates quarterly visits for the following services: Medical, Dental, Vision, and podiatry services. Ensures with each visit, all the proper paper work is completed and returned to the appropriate person. Stays available on day of visit if there are questions. Reviews psychiatric progress notes from contracted provider weekly, informing Director of Nursing of any issues. Files notes, keeping current and previous year readily available. Keeps in contact with administrator for any resident or family complaints/grievances. Ensures issues are handled by the appropriate departments. Maintains complaint and grievance log. Researches, creates, and implements educational information for staff regarding resident's rights, abuse and neglect, customer service, person centered care, etc. on ongoing basis or as needed. Serves as liaison between facility and hospital social workers/discharge planners. Assists in verification of financial eligibility and other admission paperwork. Advises and assists residents and families with community services and completing admission paperwork. Provides tours of the facility. Coordinates and manages resident/patient referrals. Communicates with facility staff to manage incoming admissions. Reviews clinical and financial documents for admission criteria. Communicates with physicians and hospital case managers to coordinate referrals. Provide education to families, potential residents, and referral sources regarding facility services. Meet with families and residents to discuss admission process and completes admission paperwork with family discussing facility policies and procedures of the resident's stay. Develops and maintain a network of referral sources to meet census goals. Distributes and presents marketing materials, hosts/participates in community events. Manages inventory of marketing materials and adhere to set budgets. OTHER DUTIES Performs other duties as required. Assists residents with shopping for needs and special requests. Serves on the NH QAPI Committee. Makes visits to area referring hospitals as needed. PHYSICAL DEMANDS Prolonged periods of standing and walking Must be able to see with corrective eye wear Must be able to hear clearly with assistance May be exposed to infectious and contagious diseases Able to handle emergency and/or crisis situations May be required to wear protective equipment as necessary Transferring, repositioning patients. Reaching, stooping, bending, kneeling, and crouching for patient care functions. Ability to perform effectively in a stressful and fast-paced environment. Ability to pass all required health and other screening tests including random and reasonable suspicion drug screens.
