RN Hospice Case Management Job Details Location: Grand Junction, Colorado 81506 Assignment Length: 13 weeks Start Date: November 10, 2025 Employment Type: Travel Nursing, On-Site Hours per Week: 36 Shifts per Week: 5 Shift Duration: 8 hours (8:00 AM - 4:30 PM) On-Call: Required at least every 3 weeks Estimated Weekly Pay: $2470 - $2810 Registered Nurse Qualifications Active, unrestricted Registered Nurse (RN) license in Colorado or compact state CPR certification required Associate's or Bachelor's Degree in Nursing or related field Minimum 1 year of RN home care or hospice experience (HCHB experience preferred) Proficiency with OASIS documentation, wound care, foley catheter management, IV administration, patient teaching, and clinical assessments Case Management certification preferred Strong analytical, organizational, and decision-making skills RN Case Management Responsibilities Conduct comprehensive assessments of high-risk hospice patients Collaborate with primary care providers to develop and implement individualized care plans Monitor and ensure cost-efficient, appropriate levels of patient care Coordinate non-medical support services such as housing and transportation to support treatment compliance Manage care transitions and coordinate with physicians, social workers, discharge planners, and claims professionals Engage specialty healthcare resources as needed for optimal patient outcomes Maintain detailed records of clinical, functional, and fiscal outcomes Identify opportunities for health promotion and illness prevention Advocate for patients, ensuring privacy and confidentiality Prevent adverse patient occurrences and intervene promptly as needed Apply now to join our Grand Junction, Colorado healthcare team as a Travel Hospice Case Management Registered Nurse.
