Florida Department of Health Office of Information Technology Formal Scope of Work Applications Development Analyst SOW24-145 1. PURPOSE: This scope of work is for Application Development Analyst services to assist the development, maintenance, and support of responsive, front-end related website elements (Velocity Script, XSLT, HTML/XML, CSS, JavaScript), Cascade Server Web Content Management System (CMS) specific site templates, .NET API web content delivery applications, a .NET TLS/SSL auto-renewal application, and system assets necessary to support the Department's public facing internet websites for the Web Administration Team in the Florida Department of Health's (Department) Office of Information Technology. Contractor will provide these services to the Department. 2. TERM: This scope of work will begin on 7/1/2024 or the date on with the purchase order is issued, whichever is later. It will end at midnight, Eastern Time on 6/30/2027. The State of Florida's performance and obligation to pay under this purchase order and any subsequent renewal is contingent upon annual appropriation by the Legislature and satisfactory performance of the Contractor. 3. LOCATION OF WORK: The worksite for this scope of work is the following location(s): Department of Health Office of Information Technology 4052 Bald Cypress Way Tallahassee, FL. 32399 4. CRIMINAL BACKGROUND SCREENING: The Department will conduct a criminal history record check, including fingerprinting, on the consultant assigned by the Contractor prior to the consultant commencing work for the Department. The Department retains sole discretion as to whether the results of the criminal history record check will result in the Contractor's employee being disqualified from performing services pursuant to this agreement. 5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE: Contractor staff assigned to this agreement must possess the following minimum qualifications and experience: 5.1. 10 years of front-end related web development experience utilizing HTML, CSS, JavaScript, jQuery, AJAX, Responsive Web Design, and .NET C# development with API and SQL Server database queries. 5.2. 3 years of experience with modifying, developing, designing, and maintaining front-end related website elements to meet design and change specifications. 5.3. 1 year of experience with creating system documentation and training. 5.4. 2 years of experience working with the Cascade Content Management System (CMS). 6. CONTRACTOR RESPONSIBILITIES: 6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified: 1 1/29/2024 5/22/2024 1:04 PM VMM Florida Department of Health Office of Information Technology Formal Scope of Work Applications Development Analyst SOW24-145 6.1.1. Participate in weekly planning sessions for task and change management planning, execution, and communication as directed by the Department. 6.1.2. Create and maintain system designs for websites and web infrastructure enhancements for existing websites and website components, daily, as specified in and reviewed during the weekly planning sessions as directed by the Department. 6.1.3. Modify and create new Cascade Server page templates, Velocity Script/XSLT formats, workflows, data definitions, configuration sets, content types and reports, daily, as specified in and reviewed during the weekly planning sessions as directed by the Department. 6.1.4. Troubleshoot, resolve, and document issues and bugs in Cascade, Node or web-related systems, daily, as specified in the work tickets assigned by the department. Tickets will be issued with due dates and must be completed within the time frame specified for a weekly audit and review as directed by the Department. 6.1.5. Document coding modifications and system changes in the assigned documentation repository, daily, as directed by the Department. 6.1.6. Assist in the creation of training manuals, step-by-step tutorials, and interactive video modules for new website components, feature enhancements, and website modifications, daily, as specified in and reviewed during the weekly planning sessions as directed by the Department. 6.1.7. DATA SECURITY AND CONFIDENTIALITY TASK: The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentiality document attached to the purchase order. 6.2. DELIVERABLES: Contractor will complete and submit the following deliverables to the Department in the time and manner specified: 6.2.1. Monthly: Provision of Application Development Analyst services in the time and manner specified in Tasks 6.1.1. through 6.1.6. 7. METHOD OF PAYMENT: 7.1. A purchase order will be issued to the Contractor. 7.2. The method of payment for this purchase order is unit rate. 7.3. The Contractor will not receive payment in advance for goods or services described in this scope of work. 7.4. The Contractor must submit an invoice monthly that provides a detailed accounting of the deliverables performed during the invoice period for which payment is being requested. 7.5. PERFORMANCE MEASURES AND FINANCIAL CONSEQUENCES: All deliverables and related tasks must be completed 100% as specified. Failure to satisfactorily complete or submit a deliverable in the time and manner specified will result in a financial consequence as indicated below: 8.1. Failure to complete and submit Deliverable 6.2.1. in the time and manner specified will result in a payment reduction equal to 15 percent of the total invoiced amount. 8.2. Failure to comply with Task 6.1.7., DATA SECURITY AND CONFIDENTIALITY will result in a payment reduction equal to 10 percent of the total monthly invoiced amount for each week the Contractor is not in compliance. 9. CONTRACTOR TRAVEL REIMBURSEMENT: The Contractor will not be reimbursed for any travel expenses under this agreement. 10.
