Massapequa, NY 11758 Shift timing:- 8:00am - 3:15 pm Student Details: Name of Student: Olivia Stupnikov Date of Birth: 11/27/2020 Grade: kindergarten Diagnosis: Type 1 Diabetes Mellitus Duties: Assist the student in monitoring their medication and overall well-being, including behavior associated with diabetes.Description: Regularly monitor the student's blood glucose levels and respond appropriately to fluctuations. Administer insulin or other prescribed medications as needed, following established care plans. Ensure that the student's medication is administered accurately and on schedule. Communicate with parents/guardians and healthcare providers about the student's medication needs and any changes in condition. Observe and assist the student with behaviors that may be related to blood sugar fluctuations, ensuring their comfort and safety. Provide support and reassurance to the student during episodes of hypoglycemia or hyperglycemia. Work closely with teachers and school staff to ensure the student's needs are met in the classroom environment. Keep emergency contact information and supplies readily available at all times. Collaborate with school staff, parents/guardians, and healthcare providers to ensure comprehensive care for the student. Participate in meetings and provide updates on the student's health status and care plan. Educate school staff on the student's condition and how they can support the student's needs. Certification: Active LPN license
