Advanced Home Health RN | Hospice Care Specialist

Position Overview: We are seeking a compassionate and skilled Registered Nurse to provide home health and hospice care services in a supportive, team-oriented environment. This role involves planning, organizing, and directing care based on physician treatment plans, ensuring patients and their families receive personalized and respectful care within their homes and communities in the Midwest region. Key Responsibilities: As a professional nurse, you will coordinate home care and hospice services, collaborate with community resources, and may assist with public health initiatives. You will also serve as an RN case manager for hospice clients, advocating for their needs and providing holistic support. Education Graduation from an accredited nursing program is required. Current licensure as a Registered Nurse in the relevant state is essential. Experience While two years of professional nursing experience is preferred, motivated and self-directed candidates with less experience are encouraged to apply.
