Field Care Manager, Behavioral Health (Social Worker) - Michigan Help improve lives by guiding members toward better health outcomes. Judge Healthcare is seeking a Field Care Manager (Behavioral Health) to support Medicare/Medicaid members through in-home and community-based care coordination. Location: Remote (home office based) Travel required: 15-20% within Wayne or Macomb County, MIMust reside in Michigan and be able to drive to assigned areasAbout the Role: As a Field Care Manager, you'll assess members' physical, environmental, and psycho-social needs to develop care plans that promote wellness and independence. You'll serve as the primary point of contact for the Interdisciplinary Care Team (ICT), coordinating services and advocating for optimal outcomes. Responsibilities: Manage a caseload of Medicare/Medicaid members through in-home, community-based, and telephonic assessments Develop and modify individualized care plans in collaboration with members, caregivers, and providers Provide clinical support and guidance to members with medical complexity Coordinate services across social, housing, educational, and healthcare domains Conduct medical attestations and clinical oversight reviews Ensure services are delivered in the least restrictive setting Serve as the lead coordinator for the ICT, ensuring member needs are met Minimum Qualifications: Bachelor's degree in Social Work (BSW) with active Michigan licensure or limited license with intent to obtain full licensure 2+ years post-degree clinical experience in behavioral health Experience with adult populations and disease management Strong knowledge of community health and social service resources Excellent communication and interpersonal skills Ability to work independently and manage time effectively Proficiency with electronic medical records and Microsoft Office (Word, Outlook, Excel) Valid driver's license, car insurance (minimum 100/300/100 coverage), and access to a vehicle Dedicated home office space with privacy and high-speed internet (DSL or cable; satellite not permitted) Full-time availability (Monday-Friday, 40 hours/week) Must pass TB screening (patient-facing role) Preferred Qualifications: Master's degree in Social Work (LMSW or LCSW) Experience with in-home assessments and care coordination Background in health promotion, coaching, and wellness Experience with Medicaid Long Term Care and managed care Bilingual in Spanish, Arabic, or Chaldean Neo-Aramaic Certification or training in Motivational Interviewing Why Join Us?
