Job Purpose: Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. The LPN/RN will report directly to the Director of Nursing. Skills/Qualifications: LPN must be 21 years of age with a HS diploma and a Certificate of Practical Nursing and/or Associates Degree, and have a current unrestricted license to work in the state of Missouri and CPR certified. The LPN/RN must have a minimum of 2 years of direct patient contact with either a Skilled Nursing Facility or an In- Home Care Agency. The LPN/RN have Clinical Skills, Bedside Manner, Infection Control, Nursing Skills, Physiological Knowledge, Administering Medication, Medical Teamwork, Multi-tasking, Listening, Verbal Communication, Health Promotion and Maintenance Job Responsibilities: Provide skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs and preventive care. Utilize a skilled approach in the provision of problem-specific therapeutic interventions, teachings and training activities in accordance with the plan of care. Develops sales by making initial presentation; explaining product and service and by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, community stakeholders on Home Care Services and the admission process. Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care. Provide hands-on care and patient education utilizing At Home Care Policy. Treat wounds, change wound dressings, giving injections, administer enema Collect specimens from patients Administers medications and set up medication dispensers. Measures and records vital signs Reports and observed changes in patient conditions to the Director of Nursing
