Job Title: Registered Nurse (RN) - Case manager Location: Atlanta, GA 30305 Duration: 3+ Months (Possible Extension) Shift: 8 AM to 5 PMUnit: Mental Health/Psych departmentLicense/Certificate: Active Georgia RN/compact license BLS - AHA Required Experience: Current 1 year of RN case management experience in Psych/Mental Health setting Schedule: Rotating weekends Summary: RN Case Manager supporting the Mental Health/Psych unit, providing coordinated patient care, assessments, and ongoing case management for behavioral-health patients. Responsibilities: Conduct comprehensive mental-health assessments and develop individualized care plans for assigned patients. Coordinate treatment services, referrals, and follow-up across multidisciplinary behavioral-health teams. Monitor patient progress, ensure continuity of care, and provide crisis intervention as needed, including rotating weekend coverage.
