Position Overview: The In-Flight Paramedic provides advanced medical care, medical oversight, and documentation management during prisoner air transport missions for the U.S. Marshals Service (USMS) / Justice Prisoner & Alien Transportation System (JPATS). This role requires rapid mobilization (within 2 hours), extensive travel, working on secure aircraft, and delivering medical care for 1-150 prisoners per mission, including stable, unstable, and complex cases. Responsibilities include emergency response, medical screening, chain-of-custody management for medical documentation and medications, and ensuring fitness-to-fly decisions in collaboration with JPATS medical authorities. High school diploma or equivalent. Valid state driver's license with a clean driving record (past 5 years). NREMT-Paramedic certification (current). CPR/AED certification. ACLS certification. Minimum 3 years of verifiable paramedic experience. Ability to read, write, and speak English fluently. Ability to pass USMS background investigation and ongoing suitability requirements. Ability to meet physical and medical fitness standards required for flight operations. No disqualifying convictions (e.g., domestic violence, moral turpitude). Critical Information: Must pass all USMS background and suitability requirements (Tier 1). Must respond to mission changes and report for duty within 2 hours, 24/7/365, including holidays. Must be physically capable of prolonged activity in an aviation and law-enforcement environment (standing, walking, climbing, lifting, responding to violent or unstable individuals). No weapons permitted - strict prohibition under USMS/JPATS policy. Requires strict adherence to confidentiality, security, medical ethics, and federal conduct standards. Skills: Strong clinical judgement and emergency response capability Excellent communication and teamwork in law-enforcement environments Ability to remain calm and decisive during stressful, dynamic conditions High attention to detail (documentation, medical inventory, compliance requirements) Organizational discipline and adaptability for unpredictable schedules Professionalism, integrity, and adherence to strict ethical standards Key Responsibilities Deliver high-quality medical care to prisoners and, when necessary, aircrew and personnel. Administer medications, medical treatments, and interventions per written or verbal orders. Perform pre-flight checks to ensure all medical equipment, medications, and supplies are present and functional. Maintain custody, chain-of-custody, and control of medical records, medications, and medical equipment. Conduct medical screening and determine fitness-to-fly based on USMS/JPATS medical standards. Provide emergency response for seizures, cardiac events, mental health crises, injuries, and other in-flight medical conditions. Prepare and deliver medical summaries to receiving officials at mission destinations.
