Position Summary: Implements the agency program of full time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor in accordance with agency standards and policies. Essential Functions and Responsibilities: 1. Initiates and provides medically prescribed skilled nursing care and/or health counseling that contributes to the treatment of the medical condition, achievement of the rehabilitation potential, and/or reduction of pain and other symptoms. 2. Makes the initial visit to assess the patient's physical and emotional status, needs, and resources through observation, interview and analysis of records. 3. Prepares the nursing plan of care, in accordance with the physician's plan of treatment, based on problems identified in the nursing assessment and provides patient teaching using the Care Plan/Standard of Care. Requirement description : Will be covering the Davison area. This is an on-call position, but they will get full-time hours by being called in at 1.1 multipliers. 2+ Years of experience Home Health and Hospice - Required Please have drivers license included in submission - Required Travel - Preferred Home Care Home Base - Required Infusion and Wound care - Required Oasis - Preferred Must live or stay within Davison Area: Genessee County, Shiawassee County, Lapeer County, Oakland County, or Livingston County- Required Health system-wide profile standards References NOT required. A skills checklist completed within one year of application for RN specialties is required. All other professions outside of Nursing are not required but preferred. Michigan license verification is required for nurses. Please provide LARA and Nursys; if there is a license hit, it would be in the traveler's best interest to provide a write-up of what happened for the manager's review. Physical copies of certifications are only required in the profile if listed above. Full-time employees of McLaren are not eligible for contract hire within six months of employment.
