SLP (Speech-Language Pathologist)

Job Summary (List Format) - Speech-Language Pathologist (SLP): - Assess, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders in skilled nursing facility residents. - Develop and implement individualized treatment plans tailored to residents' needs and physician orders. - Provide direct therapy services to improve communication, cognitive-linguistic function, and safe swallowing. - Conduct and interpret swallow assessments; recommend diet modifications as needed. - Document evaluations, treatments, progress notes, and discharge summaries in compliance with regulations. - Educate residents, families, and staff on therapy plans, communication strategies, and safe swallowing practices. - Participate in interdisciplinary care planning meetings, contributing to resident-centered goals. - Monitor resident progress and adjust treatment plans accordingly for optimal outcomes. - Maintain compliance with professional, legal, and regulatory standards. - Supervise Clinical Fellows (CFYs) and/or Speech-Language Pathology Assistants (SLPAs) as required. - Work collaboratively within a team-oriented skilled nursing/long-term care environment.
