Key Responsibilities: Supervise clinical programs to ensure high-quality, cost-effective services aligned with strategic goals. Provide guidance on clinical and case management decisions to enhance excellence in end-of-life care and symptom management. Support staff development through home visits, Clinical performance evaluations, and personalized development plans. Ensure compliance with regulatory policies and maintain a focus on positive clinical outcomes. Monitor team performance metrics to achieve desired operational measures. Use clinical software to oversee patient progression and identify educational opportunities. Facilitate the creation and update of individualized care plans for each patient. Coordinate care among all physicians and service providers involved in patient care. Manage staffing needs, from hiring to training and evaluating team members. Anticipate and respond to customer needs with speed and courtesy. Qualifications: Registered Nurse with a license to practice in Connecticut. Diploma in nursing; Bachelor’s degree preferred. Experience in hospice care, OASIS assessments, and case management certification is a plus. Proficient in keyboarding and managing electronic communications.
