Locations: Multi-Site (Traveling between Germantown, Columbia, and Nottingham, MD)Schedule: Full-Time, Monday - FridayPosition Type: Clinical / Preventive Care Position Summary Our clinic is seeking a proactive and independent LPN to spearhead our Medicare Annual Wellness Visit (AWV) program. In our unique Two-Part Wellness Model, the LPN acts as the primary clinical lead for the first encounter. You will be responsible for conducting the patient-facing screenings and initiating lab orders, ensuring that when the patient meets with their physician a week later, all clinical data and results are ready for a meaningful consultation. The ideal candidate enjoys working with the geriatric population, is highly organized, and is comfortable traveling between our three regional hubs in Germantown, Columbia, and Nottingham. Key Responsibilities (The Two-Part Workflow) Independent Visit Lead: Conduct the initial Phase of the Medicare AWV, including the Health Risk Assessment (HRA), cognitive screenings (Mini-Cog), depression screenings (PHQ-9), and functional/fall risk assessments. Clinical Data Management: Update medication lists, surgical history, and the list of current medical providers. Lab Coordination: Utilizing standing orders, initiate required preventive lab work (A1C, Lipids, etc.) so results are available for the follow-up physician visit. Closing Gaps in Care: Identify missing preventive services (mammograms, colonoscopies, immunizations) and initiate necessary referrals. Travel & Coordination: Manage a rotating schedule between our Germantown, Columbia, and Nottingham locations to ensure all patient populations are served. Documentation: Ensure all Medicare requirements are documented accurately in the EMR to support "incident-to" billing and quality reporting (HEDIS/MIPS). Smooth Transitions: Prepare the "Personalized Prevention Plan" for the physician to review and sign during the patient's follow-up consultation. Qualifications Education: Completion of LPN nursing training from an accredited school. Licensure: Active Maryland LPN license in good standing. Experience: Minimum of 1 year of experience in Primary Care, Geriatrics, or Internal Medicine preferred. Knowledge: Familiarity with Medicare Annual Wellness Visit G-codes (G0438/G0439) is a plus, but training will be provided. Skills: Proficiency with Electronic Medical Records (EMR). Strong interpersonal skills and the ability to explain the importance of preventive health to seniors. Travel: Must have a valid driver's license and reliable transportation to travel between the three clinic sites. What We Offer Competitive compensation and benefits package.
