Join Our Compassionate Team and Make a Difference! Are you an experienced and compassionate nursing professional looking to make a meaningful impact in the lives of patients and their families? We invite you to consider a rewarding role as a Hospice RN Liaison, where you will play a critical role in ensuring seamless hospice care and support for those in need. Key Responsibilities: Respond promptly to referrals from hospitals, home health agencies, physicians, case managers, skilled nursing facilities, and other healthcare providers. Conduct thorough assessments to determine patient eligibility for hospice services, reviewing medical records and engaging with physicians and family members. Educate families and patients about the scope of hospice care, its processes, and its significance with empathy and clarity. Ensure all documentation is accurate and manage the intake and admission process while meeting legal and clinical requirements. Act as a vital link between referral sources and our dedicated hospice care team, facilitating smooth transitions and open lines of communication. Make regular visits to hospitals and other facilities to maintain visibility and strengthen relationships with discharge planners, case managers, and physicians. Develop and nurture relationships with referral partners and explore new outreach opportunities to elevate awareness of our hospice services. About You: Education: Licensed Nurse with 3-5 years of hospice experience is required. BSN with 5 or more years of hospice experience is preferred. Experience: Minimum of 3 years of hospice experience is mandatory. Experience in discharge planning, case management, or education is advantageous. Licenses & Certifications: Current RN license in the state of practice. CPR certification. Valid driver's license and automobile insurance. Skills & Knowledge: Deep understanding of hospice care and the specific needs of patients and families. Familiarity with community resources and referral networks. Exceptional patient assessment and documentation abilities. Proven capability to supervise staff, manage services, and effectively coordinate programs. Strong communication skills with an appreciation for diverse backgrounds. Personal Qualities: Empathy and sensitivity toward the needs of terminally ill patients and their families. Ability to adapt to shifting priorities and manage multiple tasks efficiently. Professionalism, collaboration, and tact while working with diverse teams. Ability to educate and influence referral sources, patients, and families regarding hospice options. Strong commitment to patient advocacy and delivering quality care.
