Job Description

You'll join a compassionate, fast-growing home health and home care team that supports patients one-on-one in their homes and communities. Expect to be hands-on - providing personal care, supporting daily living needs, and making a real difference in someone's day, every shift. This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. Licensed HHAs/CNAs are encouraged to apply - and we also welcome motivated candidates willing to become certified. Why this role matters This is a high-impact, patient-facing caregiving role. You'll be a trusted presence in the home - helping clients stay safe, comfortable, and independent. Your work directly supports quality of life for patients and peace of mind for their families. Key responsibilities • Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting • Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan • Prepare meals and assist with feeding as needed • Support medication self-administration through reminders (no independent med administration) • Monitor and report patient condition and changes to the supervising nurse • Maintain a clean, safe, and orderly client environment • Assist with linens and light client laundry as appropriate • Use safety equipment properly (rails, supports, stools, etc.) • Document care provided according to company and regulatory standards • Travel reliably to patient homes and assigned visits across your service area • Complete required in-service training Must-have requirements • Age 18 or older • Active HHA or CNA certification or willingness to obtain certification or 1 year experience is the last 2 years • Willingness to complete competency and skills assessments • Ability to follow written care instructions and document services • Compassion, patience, and professionalism with vulnerable populations • Ability to work independently in home settings • Reliable transportation and ability to travel to client homes • Ability to lift up to 25 pounds • Willingness to complete background screening, and onboarding requirements If you're ready to make a daily difference in someone's life - we'd love to meet you.
