Incident Review Specialist - health and human Services

Human services professionals are encouraged to consider an Incident Review Specialist position with Liberty Healthcare.As an Incident Review Specialist, you will be an active contributor with a large-scale program that seeks to protect and ensure the quality of services and supports provided to older adults and people who are living with developmental disabilities.You will be an integral part of Liberty Healthcare’s agile team which focuses on initiatives related to critical incident investigations and reporting; continuous quality management; data systems; and HCBS provider oversight, technical assistance, and training.Your typical workday in this role will involve investigating reported adverse incidents related to the home and community based care of people who are living with disabilities.Your specific responsibilities will include: Collaborating with providers to ensure proper follow up and quality control Facilitating root cause analyses and writing reports Researching prior incidents and identifying trends Communicating findings, identifying risks, and escalating incidents to appropriate parties Training providers and providing technical assistance Oversight and monitoring of corrective action plans related to adverse events Identifying opportunities to improve provider capacity and positively impacting outcomes for persons-served The downstream impact of your work will contribute to the advancement of systemic quality improvement initiatives that positively impact the health and well-being of a significant number of older adults and people who have intellectual and developmental disabilities.As a valued full-time employee of Liberty Healthcare and Incident Review Specialist, you can expect: Competitive compensation Health insurance, dental insurance, vision insurance, life insurance, and disability insurance 30 days of paid time off each year for holidays, vacation, sick leave, and personal days Health Savings Account, 401k plan, and other perks Full details on Liberty's benefits plan are available at You are a good fit for this position if you possess the following: Bachelor’s degree – ideally in Social Work, Special Education, Psychology, Criminal Justice, or another social / health / human services field Highly organized, self-motivated and able to work well with a high degree of autonomy Strong verbal and written communication and analytical skills Ability to develop rapport with a wide array of personalities and maintain composure during challenging situations You have relevant work experience and transferrable skills if you’ve worked as an Assessor Reviewer, Social Worker, Investigator, Service Coordinator, Case Manager, Case Worker, Program Specialist, Human Services Specialist, Care Navigator, Client Advocate, Detective, law enforcement professional, or Quality Specialist / Quality Improvement professional.Liberty Healthcare is an equal opportunity employer committed to diversity, equity, and inclusion.
