LPN - Hospice - PRN - Days (69712)

JOB SUMMARY Under the direction and supervision of the Hospice R.N. Case Manager, provides skill nursing care, personal care, emotional support, and assistance with activities of daily living to terminally ill patients in the home, the majority of who are over the age of eighteen. This position is responsible for assisting in the implementation of the patient's care plan and assisting the patient in maintaining good personal hygiene and a safe and clean environment. JOB QUALIFICATIONSEducation: Graduate from an accredited vocational school of nursing. Licensure: Must have a current driver's license, valid automobile insurance coverage, and current LPN license in the State of Georgia. BLS CPR required Experience: Preferably one-year experience in an institutional setting or provided care to primarily geriatric patients. Skills: Demonstration of ability to complete personal, environmental and basic skilled and unskilled nursing care to include the following; vital signs, S & A's, foley catheter insertion, irrigation; administration of fleet and soap suds enema, infection control techniques. Administration of IM & SQ injections, dressing changes, monitoring pain and symptom control. Knowledge of organizational policies, procedures, regulations, and medical equipment utilized in patient care. Knowledge of common safety hazards and precautions for a safe work environment. Skill in modifying documentation to meet intermediary requirements. Skill in preparing, maintaining and writing reports. Skill in maintaining effective working relationships with associates, patients, medical staff and the general public. Ability to interpret, adapt, and apply guidelines and procedures. Ability to communicate clearly. Ability to work independently in patients' homes without direct supervision. Ability to drive self to various locations on a daily basis. PHYSICAL, MENTAL, ENVIRONMENTAL AND WORKING CONDITIONS Varies with patient's home environment. Moderate road and environmental hazards. Requires moderate mental effort in completing paperwork. Requires moderate amount of sitting while driving. Moderate amount of stooping, bending and lifting. Requires manual and finger dexterity and hand-eye coordination. Requires lifting up to 50 lbs. Requires corrected vision and hearing to normal range. Requires ability to distinguish letters or symbols. Requires use of office equipment.
