HIRING! RN Case Manager in San Diego CA. for Home Health and Hospice. We have several territories available. You must have home health or hospice nursing experience. Home Health & Hospice RN Case Manager role available with company that is very established, well known and reputable. This is a Fulltime Direct position with allot of opportunity for growth and advancement. Benefits Medical Insurance - Sea Coast contributes $225 per month toward employee medical premiums. Premiums are age-based, so the employee portion may vary. Dental & Vision Insurance - Available to employees as part of our benefits package/no cost. 401(k) - Offered, but no company match at this time. Possibly in the future as the company grows. Paid Time Off / Vacation - Full-time employees receive 40 hours of vacation per year after 90 days of employment. In addition, employees receive 40 hours of paid sick leave annually, 6 paid holidays, and bereavement leave (up to 5 days, with the first 3 days paid). Mileage Reimbursement - reimbursed at IRS mileage rate. This is a FT perm position with employee benefits. Below are the rates offered for this role and all rates are negotiable depending on your experience. SOC (start of care/admission): $130 Recertification: $90 Resumption of Care: $90 Billable Discharge: $65 Non-Billable Discharge: $45 Training/Admin Hours: $40 All mileage is reimbursed for drive time.
