DescriptionPosition Title: Navigation & Data Coordinator - Family Connects Chicago (FCC) Community Alignment - West Region Department: Family & Transition Services Reports to: Community Alignment Manager Position Objective and Summary: The Navigation & Data Coordinator manages FCC referral workflows and data for the West Region, ensuring that families are contacted and connected to appropriate services while maintaining accurate, HIPAA-compliant documentation and supporting data-driven program management. Essential Functions and Responsibilities: Receive and monitor referrals from FCC nurses, hospitals, and partner agencies for West Region families. Coordinate timely outreach and follow-up with families in collaboration with Community Outreach Specialists and other staff. Maintain and regularly update the West Region resource directory, including maternal/infant health, behavioral health, housing, legal, and basic needs providers. Enter and track referral, linkage, and outcome data in Unite Us and UCAN's electronic health record (Evolv) or other approved systems. Develop and run routine reports and dashboards for internal use and for CDPH reporting, including disaggregated equity indicators. Coordinate and document distribution of infant essentials and safe sleep materials, including tracking both purchased and donated items. Participate in FCC case conferences, partner meetings, and CQI activities as requested. Ensure compliance with data privacy, security, and confidentiality policies. Education/Job Experience/Certifications: Bachelor's degree in Social Work, Public Health, Data/Information Systems, or related field, or equivalent experience. Experience in case management, care coordination, program coordination, or data management in a health or human-services setting. Strong skills with databases, spreadsheets, and reporting tools; experience with electronic health records preferred. Understanding of HIPAA, confidentiality, and ethical handling of sensitive information. Detail-oriented, organized, and able to manage multiple priorities and deadlines. Strong communication skills and ability to work collaboratively with internal and external partners.
