We are seeking a compassionate and knowledgeable Case Manager to provide transition and career support services for our program participants. This role will involve offering career guidance, mentoring, and coaching to aid successful reentry into the workforce and ensure employment retention. The Case Manager will conduct follow-up services for at least one year post-release to support job retention and career progression. The ideal candidate will have a background in career counseling, job placement, or social services, with a focus on transitional populations. Key Responsibilities:- Provide individualized career guidance and support for program participants, including assistance with resume building, interview preparation, job application processes, and job search strategies. - Offer mentoring and coaching to participants to help them navigate the challenges of reentry and achieve successful employment outcomes. - Conduct follow-up services for a minimum of one year post-release to support job retention and facilitate career progression for participants. - Develop personalized action plans in collaboration with participants to identify career goals and necessary steps to achieve them. - Facilitate workshops and group sessions on career readiness topics, including job search techniques and workplace expectations. - Establish and maintain strong relationships with local employers to identify job placement opportunities for participants. - Collaborate with community resources and support services to provide a holistic approach to participant needs. - Monitor and document participant progress, adjusting support strategies as necessary to enhance success rates. - Advocate for participants within the community and assist with overcoming barriers to employment. Requirements - Bachelor's degree in Social Work, Counseling, Human Services, or a related field - Strong understanding of the challenges faced by individuals reentering the workforce, especially those with justice involvement. - Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse participants. - Proven mentorship and coaching skills, with a commitment to empowering individuals toward self-sufficiency. - Strong organizational skills and the ability to manage multiple cases effectively. - Proficiency in Microsoft Office Suite and case management software. Salary Description 75,000 - 85,000
