Collaborative hospital care environment Competitive salary and benefits Continuing education support Opportunities for clinical specialization Job Details Evaluate and assess patients with injuries, illnesses, or post-surgical conditions to determine functional limitations and rehabilitation needs Develop individualized treatment plans focused on improving mobility, strength, balance, and functional independence Provide hands-on therapeutic interventions including gait training, mobility training, therapeutic exercise, and manual therapy Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers to coordinate patient care Educate patients and families on recovery goals, home exercise programs, and injury prevention strategies Monitor patient progress and adjust treatment plans based on clinical outcomes and functional improvements Document evaluations, treatment plans, and patient progress accurately within the electronic medical record (EMR) Assist with discharge planning, including recommendations for home health, outpatient therapy, or rehabilitation facilities Ensure compliance with hospital policies, safety standards, and regulatory requirements Maintain patient safety during transfers, ambulation, and therapeutic activities Interested in hearing more?
